Capsulitis of the Second Toe: Facts, Remedies, and Stretch Exercises

here are the signs of a broken big toe joint pain

What is Second Toe Capsulitis?

At the base of the second toe, there is a cluster of tendons known as the metatarsophalangeal joint capsule (MTP capsule for short). This tendon cluster connects the toe to the bones in the foot, allowing for normal function and providing stability and balance to the entire body. However, when the MTP joint is forced to bear continuous pressure for an extended period of time, tension and inflammation can occur, resulting in a condition called capsulitis.

The primary symptoms of second toe capsulitis are pain and swelling, specifically in the ball of the foot. Some have described the condition as a constant feeling of having a rock in their shoe. Capsulitis can make everyday activities like walking or standing extremely uncomfortable or even impossible.

What causes Capsulitis of the Second Toe?

According to The American College of Foot and Ankle Surgeons, “It is generally believed that capsulitis of the second toe is a result of abnormal foot mechanics, where the ball of the foot beneath the toe joint takes an excessive amount of weight-bearing pressure.”

  • Improper fitting shoes
  • Professions that require excessive squatting or kneeling
  • Severe Bunions
  • The second toe is longer than the big toe
  • Tight calf muscles

How to Relieve Capsulitis Pain

Fortunately, second toe capsulitis is usually treatable at home. In most cases, with a proper care routine, the swelling and pain will begin to subside in a matter of weeks. Here are a few things you can try if you are suffering from capsulitis of the second toe.

  • Custom Orthotic insertsCustom orthotics are designed to provide support to areas of the foot that your everyday footwear cant. With the pressure taken off the ball of your foot, the pain should gradually decrease.
  • Ice, rest, and elevation – After strenuous activity, your MTP joints need time to rest and recover. Taking a few days to elevate and ice your feet can significantly reduce inflammation.
  • Anti-inflammatory medication – Over-the-counter oral medications such as aspirin or ibuprofen can help reduce the pain and take down the swelling associated with capsulitis

The Importance of Stretching

As mentioned above, people with tight calf muscles often also suffer from capsulitis. Adding a few simple stretch exercises into your daily routine is a great way to target two issues simultaneously. Here are two stretches you can do at home to relieve built-up tension in your legs and MTP joint capsules.

The first exercise involves standing with the balls of your feet on an elevated surface such as a stair step. Next, stand tall on your toes and slowly allow your heels to drop down, so your calf muscles are fully extended. To get the most out of this stretch, repeat this motion for at least a minute.

The next stretch is similar to the previous, except it targets each leg individually. Start by facing a wall, then place your toes against it while resting your heel on the ground. Slowly lean forward until you feel a stretch in your calves and the ball of your foot. Hold this position for 30 seconds and then repeat with the other leg.
